I will be retiring in April 2026. Serving as your physician has been one of the greatest honors of my life. I will miss you.

My medical records are maintained for six years before destruction. For no charge, I will fax relevant records to your new doctor upon your written request. Please ask your new doctor when they can receive your record (ie, do you need to be registered in their electronic medical record first?). You can sign a release form at their office, or you can provide me with a signed copy of the form on the following webpage. (This form prints more clearly directly from the webpage-the download is a little blurry). You can mail this form to me at: 3130 Chaparral Dr, Suite 202, Roanoke, VA 24018-4369. After office closure, my fax number 540-776-2919 and non-confidential email can be used for contact regarding medical record transfer: drlauramoylanrecordrequestonly@gmail.com.
I am not able to fax records to patients, only to other doctors. For fees established by the Virginia General Assembly, (handling, per page, postage), I can mail a paper copy to a patient. Contact me so that I can arrange fee collection and written authorization.
Your pharmacist may choose not to refill my prescriptions after April 4, 2026. You may be asked to obtain a prescription from your new physician. I recommend planning for this, so that you don't end up missing any doses of your chronic medications. Please let me know where to fax your record as soon as possible.
